Downloadable Forms

Most BSA National forms can be found on the BSA website at http://www.scouting.org/scoutsource/Media/forms.aspx

This page lists the most commonly needed forms, both National and locally generated. These forms are provided in a variety of formats, denoted by the following codes:

  • No format indicated (hyperlink) = you may view and print this form from your web browser.
  • PDF = Adobe Acrobat Portable Document Format (PDF). You need a copy of Adobe Acrobat Reader, which you can obtain free on the Adobe website.
  • RTF - Rich Text Format - used by Microsoft Word and some other products.
  • WFW = Microsoft Word for Windows 6/95 or 97 (WFW) format.
  • WP5 = Word Perfect 5.1 for DOS
  • WPD = WordPerfect 6.1 for windows.

Note: Wherever possible, these forms are links to BSA National's own forms site, so as to always have the most up to date forms. Forms from that site are flagged as "link to BSA site"

    Baden-Powell Council
    Combined Medical Form - MS Word  PDF - this form has been modified from the BSA form to include NYS requirements. To be used at Camp Barton or Camp Tuscarora and other B-P Council camps requiring medical forms. 
    BSA National Medical Form - the current National form, a single fillable PDF form replacing the old class 1, 2 and 3 medical forms, including parental permission forms. To be used for High Adventure and other out-of-council sites. - link to BSA site - PDF

  • Merit Badge Counselor Application link to BSA site PDF
    Note: new MB counselors must also submit an Adult registration form, even if they are already registered in some other capacity.

  • Money Earning Permit Application Required before running a unit fundraiser - link to BSA site PDF

  • Tour Permits: Both Local and National Tour Permits may be applied for online via MyScouting.org, or you can use the paper forms:
    • Local Tour Permit: Entire form in one file - link to BSA site (PDF only). For trips under 500 miles, entirely within the USA. 
    • National Tour Permit: link to BSA site - (PDF only) Used for trips over 500 miles, or outside the USA.
  • Training Knots - see Advancement Page
  • Transfer Form - Youth WFW - Adult WFW - Both PDF
  • Troop Resource Survey: PDF - survey your parents for help in your troop's program.
  • Uniform Inspection Sheet - Leaders link to BSA site - PDF
  • Veteran Scouter Award Application - PDF (Mid-America Council website link) - for applying for Veteran year pins (every 5 years). Note that pins up to 20 years are awarded by Council, 25 years and up are from National. Submit the application to the Council office in any case.

October Roundtable Notes: 

We updated our Roundtable schedule with further signups for the "show-and-tell" segment of our program. We still need people to present in February, April and May. If you did not have someone who picked up Dorene Sorenson's fabulous set of "Works of Art" ideas, please let me know and we'll get Dorene to send it around in electronic format. I thought her CD case as picture frame was inspired ...

We discussed district-wide cub events for the year. The ideas we have include:

3) January - Judy Klutch is going to check to see if we can arrange for tubing at Greek Peak as a group. We also discussed going to a Cortland and a Tompkins sledding spot as a group as a backup.

4) February - District Pinewood Derby. WE NEED HELP. I'm asking for each unit to identify a person who can help Scott Mosher with the set-up for this year's event which will coincide with the Mall Show. This job is perfect for the "trailing spouse" who can't be involved on a regular basis ... find those "handy" people in your pack parent supporters and send them to Scott!

5) March - we left ourselves a little open on this month

6) April - we started with the idea of sharing Pack 24's swim at TC3. John Sinnott's going to check to see if that's possible. We then started talking about seeing if TC3 would let us use their facilities for an idea that Scott had - a "Belt Loop Bonanza" where cubs could work on all sorts of belt loops. I'm going to check with TC3 on facility availability ... and we'll take it from there.

7) May - the Council Camp-o-ree. We are told there will be a cub component at this 100th anniversary event at Greenwood Park in Broome County. There should be at least 350 boy scouts there ... if it is anything like the last Council camporee it will be an exciting experience.
